Report something that needs action — from the field, from a job, or straight from a chat message.

Defects & issues is one shared spine for every nonconformity: "Report, review, action, evidence, close, audit." Issues get onto it several ways.
From the field
On site, tap Capture → Defect or issue ("Report something needing action."). The Report issue dialog opens with the record you're on already set as the subject. Fill in:
- Title (required), Severity (Low / Medium / High / Critical — or none), Category and Description.
- Add photo — evidence up to 25 MB.
You can raise an issue anchored to the job you're working, and it's logged against that job automatically.
From a chat message
Spotted a problem in Chat? Promote to record → Raise as issue turns the message (and its photo, as evidence) straight into a defect — the title defaults to the message text, and the description carries the full message.
Automatically from inspections
If an inspection form is set to raise a defect for failed items, each failing item auto-creates an issue titled "Inspection failure: {item}", linked back to the submission — so a failed check becomes an actionable issue with no extra step.
Who can raise
A field worker anchored to the subject, or anyone who manages compliance, can raise an issue. Every raise notifies the compliance managers ("Issue reported: {title}").
Tip: describe what you see, and let severity be graded properly afterwards — a clear title and a photo are worth more than a guess at how serious it is. What happens next is in The issue lifecycle.
